New patients, please complete the new patient forms package online before your first visit. There is nothing to print or download. Your answers are sent securely to our office, and any changes can be made at your visit.
What to bring
- A photo ID
- Your dental insurance card
- A list of the medications you take
- Your physician’s phone number
Dental insurance
We participate with most dental insurance plans, including MetLife, Guardian, Aetna and Cigna. If you do not see your plan, call us and we will help. For exact details about your coverage, please contact your insurance company.
No insurance? Our in-office discount dental plan includes exams, cleanings and X-rays, plus 20% off eligible treatment.
